You will need to submit this form (which you may request from your HSA service provider), along with your receipt to your HSA Plan provider for reimbursement.

"I really like this cream. I used it 3 times a day during my 5 weeks of radiation, especially for moisturizing, in combination of a prescription cream, pure aloe vera and Polymem pads. I still use the cream daily and it's a wonderful non-greasy moisturizer and healing cream. The only negative that I experienced was, that 3 out of 8 tubes I purchased, the caps broke off in the first couple of days of use. That's a problem if you want to take it in your bag, which I did during radiation. (I used it immediately after treatment for relief)"
